Requirements:
1. For 18-21 years old
*Birth Certificate (NSO Copy)
Latest original copy of Baptismal (if no record of Birth)
*Valid ID with address in Manila (Original and Xerox Copy)
*Parental Consent
Note: Parents must come personally with valid ID (Original and Xerox)
*Marriage Counseling (Manila Health Department & MSWD)
2. For 22-24 years old
*Birth Certificate (NSO Copy)
Latest original copy of Baptismal (if no record of Birth)
*Valid ID with address in Manila (Original and Xerox Copy)
*Parental Advice
Note: Parents must come personally with valid ID (Original and Xerox)
*Marriage Counseling (Manila Health Department & MSWD)
3. For 25 years old and above
*Birth Certificate (NSO Copy)
Latest original copy of Baptismal (if no record of Birth)
*Valid ID with address in Manila (Original and Xerox Copy)
*Marriage Counseling (Manila Health Department Only)
Steps:
1. Request for Order of Payment (Php 120)
2. Attend Seminar/s
3. Submit all the documents at the counter for the Application for Marriage License.
4. Release of Marriage License is after 10 days
Marriage License is valid for 120 days.
Reminder: Please observe proper attire during oath taking (Avoid wearing shorts , sandos and slippers)
